Set up a 10 minute call with the therapist to discuss concerns and next steps.

Complete Intake Paperwork
Log into our portal and complete all of the forms and upload your insurance card.
4.
Schedule Evaluation
Once insurance eligibility has been verified, the therapist will reach out to schedule your child's 45-60 minute evaluation.
